Registration Forms must be completed and turned in by every student each year, prior to the start of Band Camp. Forms Packets will be printed and distributed at Forms Nights, in addition to being available here for download.
Permission Slip Packets will be posted for each season, and must be turned in prior to season start.
All above forms that require a signature must be printed and signed. You may then send hard copies in with your student, which they will place in the Blue Box. Due to the number of forms and the number of students, please do not email forms back.
